HOW TO REFER
Referrals to the Unit come from a variety of sources, including Student Welfare Coordinators, Principals, Guidance Officers, Social Workers and Youth Workers. Verbal contact initially determines the appropriateness of a referral.
A referral form is forwarded to the referring agent and is completed in consultation with the student's parents/guardians and school. Schools are asked to form a Student Support Group, if one does not already exist, which prepares an Individual Learning Plan for the student. Students and parents are encouraged to visit the Unit for an introductory chat with a staff member.
From term 2 2003, a "Staged Response Proforma" completed by the student's regular school will also be required before a referral is accepted.
Students are placed on the Unit's waiting list when a completed referral form is received. When a place becomes available, an assessment meeting with the student, his/her parents, a school representative, Unit staff and relevant others is arranged, at which the program is explained in detail and goals for the placement established. If all parties agree to the suitability of the placement a commencement date is set.
A levy of $300.00 is paid by the referring school at the commencement of the student's placement.
